Why Your Stomach Growls — Even When You’re Not Hungry

That growl isn’t your stomach asking for food. It’s housekeeping.

The noise is called borborygmi, and it is not your stomach asking for food. It is gas and liquid being pushed through your digestive tract by a cleaning cycle called the migrating motor complex, which runs roughly every 90 to 120 minutes whenever you are not eating. It is louder on an empty stomach for the boring reason that there is nothing in there to muffle it.

The hunger association is real but backwards. The growling does not signal hunger; both the growling and the hunger tend to arrive at the same point in a cycle that would have run anyway.

Where the sound comes from

Your digestive tract moves its contents by peristalsis — coordinated waves of smooth muscle contraction squeezing material along. Those contents are never purely solid. Even a supposedly empty stomach and small intestine contain swallowed air, digestive secretions and residual fluid.

Push a mixture of gas and liquid through a flexible tube by squeezing it, and it makes noise. That is the entire acoustic explanation: turbulence as gas and fluid are forced through narrowing passages. The stomach and small intestine sit close to the anterior abdominal wall with little between them and the outside, so the sound carries.

The housekeeper wave

The interesting part is the timing, and it has a name. The migrating motor complex is a cyclical pattern of electrical and motor activity that sweeps through the stomach and small intestine during fasting, first characterised in the late 1960s.

It runs in phases. A long quiet period of little or no contraction, then a phase of irregular contractions, then a short burst of strong, regular, propagating contractions, then a brief transition back to quiet. The whole cycle takes roughly 90 to 120 minutes and repeats for as long as you are not eating.

The strong phase is sometimes called the housekeeper wave, and the name captures its job: clearing residual food particles, secretions, bacteria and debris out of the upper digestive tract and onward. Without it, material accumulates, and impaired motor complex activity is associated with bacterial overgrowth in the small intestine.

Eating interrupts the cycle entirely. Feeding switches the gut into a different motility pattern optimised for mixing and gradual emptying, and the migrating complex does not resume until digestion of that meal is largely complete.

So the growling reflects a rhythm running on its own schedule in the background, largely independent of what you are consciously doing — the same category of autonomous timing as the hormonal ramp that prepares you to wake before your alarm.

Why an empty stomach is louder

Two reasons, and the second is the one usually missed.

First, muffling. Food and chyme absorb and damp sound. A stomach containing a meal is acoustically padded; an empty one is a gas-and-fluid resonating chamber with a thin wall between it and the room.

Second, the motor complex only runs when you are fasting. The loudest phase of the cycle is literally only available on an empty digestive tract, because eating shuts the cycle down. So the association between emptiness and noise is not merely acoustic — the noisiest activity is a fasting-specific behaviour.

Your gut is not silent after a meal. It is working harder than during fasting. You simply cannot hear it as well, and the character of the activity is different.

The hunger question

Hunger and borborygmi correlate without one causing the other. Hunger is driven principally by hormonal signalling — ghrelin rising during fasting, along with falling blood glucose and signals from an empty stomach — and by learned meal timing.

Ghrelin also stimulates gastrointestinal motility, which is part of why the two travel together. The growl is not a request. It is a parallel consequence of the same fasting state that produces the appetite, and it will keep happening on schedule whether or not you are hungry, whether or not you intend to eat, and while you sleep.

It is worth noting that the reflex arc has no communicative function at all here — unlike the hiccup, whose evolutionary story is at least actively argued over, nobody proposes that borborygmi are for anything. They are the audible byproduct of a mechanical process that would happen silently if the tube were fuller.

The sound has a clinical life

Borborygmi is a Greek onomatopoeia, and it is a real clinical term rather than a joke — bowel sounds are routinely assessed with a stethoscope as part of abdominal examination.

What clinicians listen for is not loudness but character and presence. Normal bowel sounds occur intermittently at a rate of several per minute. Absent sounds over a sustained period can indicate that motility has stopped, which is a genuine concern. High-pitched sounds with a particular tinkling quality can suggest obstruction, with the tract working hard against a blockage.

So an audible stomach in a quiet room is, from a clinical standpoint, the reassuring version. The sound means the tract is moving, which is what it is supposed to be doing.

Why it always seems to happen in silence

This is a perception effect rather than a physiological one, and it has two parts.

Masking: in a noisy room the sound is inaudible to everyone including you. It is happening during the meeting either way; you only notice in the quiet parts. And attention: once you are worried about it, you monitor for it, and self-monitoring makes the sound far more salient to you than to anyone else. People consistently believe their stomach noises are more audible and more noticed than observers report.

The one genuine physiological contributor is stress, which affects gut motility through the autonomic nervous system — so a tense silent situation can slightly increase the odds while also being the situation in which you would most notice.

When it means something

Ordinary borborygmi require no attention at all. Bowel sounds are a routine part of clinical examination precisely because they are expected — their complete absence is more concerning than their presence.

Loud gut noise accompanied by pain, bloating, altered bowel habits or unintended weight loss is a different picture, and one for a qualified clinician rather than an article about why your stomach is audible in a quiet meeting.
